Score Sheet

Personal data about the student is included on the score sheet.  This will enhance the examiner’s ability to place pertinent information on the assessment report.

This information includes:

bullet

Name of student

bullet

Grade

bullet

School

bullet

Disability

bullet

Test date and birth date (allowing one to determine chronological age)

bullet

Limb Involvement (specific body parts are listed for quick recording)

bullet

Mobility (specific devices are listed for quick recording).  It also includes a blank space where one could add a mobility device or state none needed.

Scoring procedures used by the test administrator are easily understood and quickly identifiable on the score sheet.

The scoring of each test item occurs immediately following the motor response of the student.  Some of the sub-test items require only a check mark, while others require a specific numerical recording. Statements are included that indicate when a specific test item should be deleted.  The Upper-Limb Coordination section has a space for identifying whether the student performed the test item from a sitting or standing position.

See Order Page for more information.
